About South Port Day Links

South Port Day Links (SPDL) provides a number of services to link frail, aged and disabled people (and their carers) with the community through its volunteer network. Funded by the Department of Human Services, the not-for-profit service is designed to provide this group with the same access as people in the general community. All its programs underpin the principle of encouraging participation in individual and social activities inside and outside the home enhancing the person’s involvement, satisfaction, autonomy and dignity.

Eligibility

Users of the service typically meet one of the following criteria:

1. Are living at home, in a boarding house or special accommodation house.
2. Because of frailty, chronic ill-health, injury or the ageing process have limited opportunities for social connections.
3. Have a physical, intellectual or psychiatric disability.
4. Are unable to access mainstream public transport because of geographical isolation.
5. Are on a low income.
6. Have a temporary medical condition and who may require service for a short period of time.

All programs are available to the frail elderly and people with disabilities (and their carers) who are residents of Garden City, Port Melbourne, South Melbourne, Albert Park, Middle Park and St Kilda West. In addition, Petlinks is available throughout all other areas of the City of Port Phillip.

How to access services Referrals are accepted from the prospective client, a family member or friend or from an organization. After initial contact, the client will be asked for information. which is used to help DayLinks provide the best service for the client. These questions can be asked over the phone and take less than five minutes. All information is protected by the SPDL Privacy policy.
Cost Services are provided free of charge, although a $3 donation is requested for transportation (either one-way or two-way). All donations above $2 are tax deductible.
Services Provided
Daylinks Community Transport
Daylinks vehicles or mini bus provides door to door transport for people of limited mobility. Drivers take people mainly within the northern part of
the City of Port Phillip to hairdressers, shopping, to attend classes and activities, for socialising with friends, and to appointments. The Daylinks community transport service will bring clients home from all activities except from hospitals and doctors out of the local area.
 
Gardenmate
Provides volunteers that enjoy gardening to assist those that are unable to tend to their gardens through age or disability. Program also provides friendship visits.
 
Walkmate
Provides a companion for those that have been encouraged to take exercise as part of their rehabilitation after surgery, stroke or heart problems, and also to those who may have lost confidence in walking on their own either following a fall or because of reduced mobility or isolation.
 
Petlinks
PetLinks volunteers walk dogs, clean out budgie cages, transport sick animals to the vet and provide assistance with tasks that help maintain  quality of life for both animal and owner.

Aged Care - What is it? We believe it is important to value older people, and that aging can be a healthy and positive time of life. We work with older people and their families to help address some of the issues faced by aging people.
Our nurses can help you by:

Managing bladder problems (continence)

Assessing and managing memory loss

Managing Medication

Setting up a safe hygiene routine

Helping you care for a frail, sick or confused older person

Helping you find and link with other community services for older people.

Which staff can help? All our nurses are skilled in providing aged care. We also have nurses who are experts in aged care. They are known as "Clinical Nurse Consultants - Aged Care". These specialist nurses work along side our general staff to ensure that all your aged care needs are met.

Further, lack of access to affordable hardware, software and support is also an inhibitor to participation.
